Who qualifies for free nutrition services?
New Yorkers enrolled in Medicaid or an eligible managed care plan who live in Brooklyn, Queens, Manhattan, or the Bronx, and who have a diet related health need or trouble affording healthy food. Final eligibility is confirmed by your plan during a free screening call.
What does it cost me?
Nothing, if you qualify. Under New York State's Medicaid 1115 waiver, eligible members receive nutrition analysis, dietitian counseling, medically tailored meals, and produce prescriptions at no cost. There is no copay, no deductible, and no bill.
What if I am not on Medicaid?
Call us anyway at (929) 460-0610. We will check whether you qualify through another plan, help you apply for Medicaid or SNAP if you are eligible, and point you to community food programs in the meantime.
Do I need a referral from my doctor?
No. You can request services yourself. If your doctor does refer you, we send the dietitian's assessment summary back to their office so your care stays connected.
Nutrition Analysis
What is a nutrition analysis?
A registered dietitian reviews what you actually eat over about three days and converts it into numbers: calories, protein, carbohydrate, fat, sodium, added sugar, fiber, and a panel of more than 40 vitamins and minerals. You get the results in writing, in plain language.
How long does the assessment take?
About 45 minutes. Most members receive their written report and a review appointment within one week.
What should I bring?
A rough sense of what you ate over the last three days, any recent lab results if you have them, and a list of your medications and supplements. If you do not have labs, we work with what you have.
Do I have to weigh or track my food?
No. Your dietitian walks you through it in conversation, using household measures like cups, spoons, and hand sizes. Nothing is weighed and nothing is logged in an app unless you want to.
Can I share the report with my doctor?
Yes, and we encourage it. The report is yours to keep. With your consent we can also send it directly to your provider.
Dietitian Visits
What conditions do your dietitians work with?
Diabetes and prediabetes, high blood pressure and heart disease, kidney disease and dialysis, pregnancy and postpartum nutrition, cancer care, digestive conditions, and unintended weight loss or gain.
Are visits in person or remote?
Both. You can be seen at our Brooklyn office, in your home, by phone, or by video, whichever gets you an appointment soonest.
Will you tell me to stop eating the food I grew up with?
No. Our dietitians adjust your recipes rather than replacing them. We work regularly with halal, kosher, Chinese, South Asian, Caribbean, and Latin cooking.
How many sessions will I have?
It depends on what your analysis finds and what your plan covers. Most members start with an initial visit and a follow up, then continue as often as the plan is helping.
Meals & Produce
How do medically tailored meals work?
Your dietitian writes a meal plan against your diagnosis and lab values, and meals matched to that plan are delivered to your home ready to eat. No cooking or shopping is required.
How many meals can I get, and for how long?
Eligible members may receive up to three meals a day for up to six months, depending on what the State and your plan approve. Your dietitian will confirm your specific amount.
Can meals fit halal, kosher, or vegetarian diets?
Yes. Halal, kosher, vegetarian, and culturally specific menus are available, alongside renal, diabetic, and low sodium options.
What is a produce prescription?
A written prescription from your dietitian for a set amount of fruit and vegetables, redeemed as a delivered grocery box or at a partner site, the same way a medication prescription works.
How soon does food start arriving?
Most members hear back within one business day, and deliveries typically begin one to two weeks after screening and the dietitian visit are complete.
Provider Referrals
How do providers refer a patient?
Use the guided form at /provider-referrals. It takes about four minutes and covers the referrer, the member, the services requested, and consent.
How fast will you contact my patient?
We review every referral within one business day and reach out to the member in English, Chinese, or Bengali, with interpreters available for other languages.
Do I get anything back?
Yes. Referrals are closed loop. You receive confirmation of outreach, enrollment status, and the dietitian's assessment summary for your records.
